In accordance with the No Surprises Act (effective January 1, 2022)
Your Right to a Good Faith Estimate
Under federal law, healthcare providers, including registered dietitians, are required to provide uninsured and self-pay clients with a written estimate of expected costs before services begin. This is called a Good Faith Estimate (GFE).
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services, including nutrition counseling, consultations, and related services. Your estimate will be provided to you before your first appointment and upon request at any time.
Who This Applies To
You are entitled to a Good Faith Estimate if you:
Do not have health insurance, or
Are not planning to use your health insurance for Dawn Anderson Nutrition services, or
Are paying out of pocket for any reason
If you are using insurance to pay for services, your insurance plan's cost-sharing terms (copays, deductibles, coinsurance) will apply instead. Contact your insurance provider directly for information about your specific coverage and expected costs.
Services and Expected Cost Ranges
The following ranges represent typical costs for services offered by Dawn Anderson Nutrition. Your individual Good Faith Estimate will reflect your specific care plan and the services we agree on together.

| Signature Course(s) | 4 to 6-week culinary nutrition program including video lessons, downloadable resources, and weekly Q&A. |

Note: Costs listed are estimates. Your actual costs may be higher or lower depending on your individual care needs, session length, and the specific services included in your care plan. A personalized Good Faith Estimate will be provided to you before services begin.
What Is Not Included
Your Good Faith Estimate covers Dawn Anderson Nutrition services only. It does not include costs for:
Laboratory tests or bloodwork ordered by your physician
Services provided by other healthcare providers
Third-party platforms or tools you may choose to use independently
How You Will Receive Your Good Faith Estimate
You will receive your individual Good Faith Estimate in writing by email or a secure client portal at least one business day before your scheduled appointment. It will include:
A description of the specific services planned
The expected cost for each service
Provider name, credentials, and contact information
Date of the estimate
Please review your Good Faith Estimate carefully before your appointment. If you have questions about any item, contact us before your appointment.
Questions or Disputes
If your final bill is at least $400 more than your Good Faith Estimate, you may be eligible to dispute the charge through the federal patient-provider dispute resolution process.
